This occurred this morning for us also. We have Time-Warner Cable.
It was present despite: Second Audio Program, Descriptive Service and Alternate Audio Track were off on cable box, DVR and television. It was present on second DVR also which is attached directly to cable.
I also have Time Warner cable here in Rochester, NY and this is an issue that has recently appeared. My TV monitor is set to stereo, not SAP, so it must be something that TW has done. They have had several service interruptions lately. I wonder if this is one of the after effects.
