I don't know if it was mentioned already but Bette Davis was the first considered choice to play the title role in Mildred Pierce. But she turned it down and they called on Joan Crawford who of course became an even bigger name after shooting the film. (Bette must have been kicking herself!)
Also as an aside, for Little Caesar with Edward G. Robinson, the Douglas Fairbanks Jr character almost first went to Clark Gable but they changed their minds because Gable apparently had "big ears" and they didn't want a gangster with big ears (???!)
Vivien Leigh was up for playing "Cathy" in 1939's Wuthering Heights so she could play opposite her beau Sir Laurence Olivier, but they gave the part to an unknown named Merle Oberon. Once again Vivien Leigh auditioned to play "Mrs. Maxwell de Winter II" opposite once again Olivier in 1940's Rebecca but Hitchcock/Selznick decided Joan Fontaine was a better fit.
Lastly Buddy Ebsen originally slotted to play the Tin Man in The Wizard of Oz and had the parted and started filming but he almost died due to fatal allergy to the silver makeup and had to spend a large amount of time in hospital in an iron lung. (Who played the Tin Man again? Was it Ray Bolger or Bert Lahr? I get the names mixed up.) Either way they went with one of those actors, who did a great job despite the fact Ebsen was the first choice because of his dancing. (I know! Uncle Jed could dance!)

Boris Karloff in Targets. He made a few films after that but they were foreign clunkers so I don't count them.
I always wanted to watch Targets. It sounds interesting...

Jean Harlow
Saratoga (1937) with Clark Gable. Not the BEST movie she's done, but very interesting to watch since she was dying, her body bloated in some scenes, emaciated in others, and an eerie foreshadowing scene where Harlow has a long, hard-to-watch coughing fit near the end of the film where Gable had to improv a couple lines.
