You were right on the first three. Leslie Caron played the wife of Anthony Hopkins in the miniseries based on Leon Uris' "QB VII."

Yes, Mike Todd, who'd been a partner in the development of Cinerama, financed the development of 70mm Todd-AO shortly thereafter. ("Oklahoma" was the first to be filmed in the process.

I realize this is obscure info, so I guess I should give some hints...

 

HINT: Only 50% of the movie was in widescreen format, and it was only shown that way in its initial release. (Some sequences were also in color.)
